DOT exam 95$ (30 min) is availabe for every Tuesday and Friday 9-12 AM, 1-4 PM. you can book online in the DOT section below, no need to call. Text or call if you need the exam on a different date. ​

The price for the I693 exam is different for different age groups due to exam requirements.

The form will be ready 3-5 days if all tests are normal.

If QuantiFERON TB test is positive, chest xray will be required, it will take additional 1-2 days to have the form ready.

We will prepare your I693 form with your information. You do not need to bring I693 form.
​
- Vaccination requirements are different for different age groups.
- Consultation for vaccines and vaccination are part of the initial appointment.
- Vaccines can be administered at my office, at a pharmacy (if you have medical insurance).
- A referral letter will be provided for local public health if you don't have medical insurance .
- MMR, hepatitis B, and varicella titer tests are available if you had those vaccinations before but do not have the vaccination record.

​5.Physical exam for head, neck, lungs, abdomen, extremities.
6. Price: 95 $ ; approximate time 20-30 mins. Check, cash and zelle are accepted.
Credit card payment is not availabe.
7. Please take a photo of your driver license and Text it to 507-7228606 prior to your appointment. I will have all information for DOT form 5875 and 5876.
