The price for the I693 exam is different for different age groups due to exam requirements.

Discount for age of 17 and younger, family discount are available

The form will be ready 2-3 days after the appointment if all tests are normal.

If QuantiFERON TB test is positive, chest xray will be required, it will take additional 1-2 days to have the form ready.

If you had a positive Quantiferon TB test in the past and have a report, you can skip this test and get the chest x-ray instead.

We will prepare your I693 form with your information. You do not need to bring I693 form.
​
- Vaccination requirements are different for different age groups.
- Consultation for vaccines and vaccination are part of the initial appointment.
- Vaccines can be administered at my office, at a pharmacy (if you have medical insurance).
- A referral letter will be provided for local public health if you don't have medical insurance .
- MMR, hepatitis B, and varicella titer tests are available if you had those vaccinations before but do not have the vaccination record. Tetenus and polio titers are not acceptable according to CDC.
